You may not require long hours of nursing care or therapy service, if this is the case, we offer our services in 30-60 minute visits that can cover your clinical or physical therapy needs with frequencies ranging from a weekly visit to a visit every day. A wide variety of physician ordered services for both nursing and physical therapy can be accomplished with our visit program. - Blood Pressure, Vital Sign check, Blood Glucose, Weight Check
- Weekly medication planner filling and medication management
- Wound Care and Dressing Changes
- IV Infusion support including PICC and Port Management
- Disease Management and Teaching including COPD, Diabetes, and congestive heart failure
- Muscle Strengthening and Stretching
- Maintenance of a Home Exercise Program
- Home safety including fall risk assessment and safe transfer and ambulation
